FEES FOR SERVICE
CANCELLATION POLICY AND BILLING OPTIONS
I am committed to providing exceptional speech and language services and want to have the opportunity to see and help as many clients as possible. Therefore, I ask that you provide me with 48 hours notice should you wish to change or cancel your appointment. You can notify me of your changed or cancelled appointment either by email or phone. If your appointment is on a Monday, please call by 2 p.m. Friday.
I understand that life happens and sometimes things come up (e.g. illness, car trouble, weather, etc.), therefore for your first no-show, I will take on the full service fee and you will not be charged. Should a second no-show occur, you will be charged 50% of the service fee. Any future no-shows will be charged 100% of the full service fee.
Payment for you or your child can be made in advance or at the end of the session. The following payment options are available: Cash, cheque or e-Transfer
Should a cheque be returned by your bank for any reason, there will be an administration fee of $25 as well as interest charges on any overdue amounts owing.
Most insurance coverage companies have an amount that can be used for Speech Language Pathology services, which I encourage you to look into. However, at this time Aspire Speech deals solely with you for billings, therefore once you have paid for your session you will be provided with a receipt in which you can seek reimbursement from your insurance provider.
Disability Tax Credit
It is possible that you or your child may be eligible for a Disability Tax Credit (DTC). Should you wish to apply for DTC, I can provide a letter outlining the areas of difficulties in which you require support to add to your documentation. Once the application is submitted the Canada Revenue Agency will determine your eligibility.
Income Tax Deductions
Did you know that fees for speech-language pathology services can be claimed as a Medical Expense on your taxes? A complete list of eligible medical expenses are available on the Canada Revenue Agency website.
Assessment includes standardized and non-standardized assessment, detailed case history, client questionnaire(s) and observations of client. If a written report of the assessment is required, there is an additional cost. An estimate on the time needed to complete the report will be provided in advance. Written documentation such as a letter request to a specialist and/or completion of the Ontario Disability Tax Credit Forms costs $30.
